1. Assumption of Risk

In consideration of my (or my child’s) attendance and participation in the martial arts training programs offered by Warrior Martial Arts Academy (“the Academy”), I, {name}, the student and/or parent/guardian, acknowledge that martial arts training involves inherent physical risks. These risks include, but are not limited to, physical contact with instructors and other participants, falls, joint locks, chokes, strikes, collisions, and use of training equipment, each of which may cause serious injury or even death.

I knowingly and voluntarily assume full responsibility for all risks of injury, damage, or loss, whether foreseeable or not, that may occur as a result of participation in martial arts activities.

2. Release and Waiver of Liability

On behalf of myself, my child, our heirs, executors, and assigns, I hereby release, waive, and forever discharge Warrior Martial Arts Academy, its owners, management, instructors, staff, representatives, contractors, and fellow students from any and all claims, demands, liabilities, actions, or causes of action arising from or related to any injury, damage, or loss sustained during participation in any Academy program or while on Academy premises, whether arising from ordinary negligence or otherwise.

This release includes, but is not limited to, claims for bodily injury, personal property damage, emotional distress, or wrongful death.

3. Physical Fitness Acknowledgment

I certify that I (and/or my child) am physically fit and have no medical condition that would prevent safe participation in martial arts training. I agree to assume full responsibility for any medical expenses that may arise from injuries sustained during or as a result of participation in Academy activities.

4. Indemnification

I agree, on behalf of myself, my child, and our successors, that should any claim be asserted in contravention of this agreement, I/we shall indemnify and hold harmless Warrior Martial Arts Academy, its management, assigned staff, instructors, and representatives from all expenses, including reasonable attorney’s fees, incurred in defending such claim, unless the Academy or its representatives are found by a court of competent jurisdiction to have acted with willful or wanton negligence.

5. Communicable Disease Acknowledgment (Including COVID-19)

I acknowledge the contagious nature of COVID-19 and other communicable diseases, and voluntarily assume the risk that I and/or my child(ren) may be exposed to or infected by such diseases while attending or participating in Academy programs. I understand that such exposure or infection may result in illness, injury, disability, or death.

On behalf of myself, my child(ren), and our heirs and assigns, I hereby release, covenant not to sue, discharge, indemnify, and hold harmless the Academy, its owners, employees, contractors, and representatives from any and all claims (“Claims”) arising from exposure, infection, or related consequences, whether due to actions, omissions, or negligence of the Academy or otherwise.

This release applies to all Academy-related activities, whether before, during, or after training sessions.

6. Severability

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect to the maximum extent permitted by law.

7. Acknowledgment and Acceptance

I have carefully read and fully understand this Assumption of Risk, Release of Liability, and Indemnification Agreement. I understand that by signing below, I am waiving substantial legal rights, including the right to sue for damages in the event of injury or loss.

I sign this agreement freely and voluntarily, intending it to be a legally binding instrument on myself, my child, and our successors.
